MISSION & VISION
Mission: Evergreen Physical Therapy is dedicated to providing quality personalized rehabilitation services, self care and wellness to the general population as well as specific individualized care in the areas of sports medicine, occupational injuries and prevention, pre and post orthopedic surgical care.
Vision: Evergreen Physical Therapy will offer patients the most optimal combination of rehab care and self care available in the community and will be the provider of choice by medical care professionals.
STAFF
Patrick Howe, DPT, MS, graduated from Central Michigan University in 1990 with a Bachelor of Science in Biology, 1998 with a Masters of Science degree in Physical Therapy and in 2004 earned his Doctorate degree from Simmons College, Boston, MA. He is currently a member of the American Physical Therapy Association and the Michigan Physical Therapy Association.
Pat has been practicing physical therapy in the Mid-Michigan area for several years. He founded Evergreen Physical Therapy in order to practice and promote physical therapy in an environment that embodies his philosophy of treatment. Pat's professional interests include orthopedics, manual therapy and sports medicine.
Pat, his wife Jamie, and his daughter Alexandra reside in Sanford. His personal interests include tennis, outdoor activities and aviation. Pat has been an active pilot for many years and has restored several antique aircrafts.
Alicia Flachs, DPT, graduated from Central Michigan University in 2004 with a Bachelor of Science in Therapeutic
Recreation and in 2007 earned a Doctorate of Physical Therapy. She is currently a member of the American Physical Therapy Association and the Michigan Physical
Therapy Association.
Alicia has been practicing in the Mid-Michigan region for several years and is currently the director of the Evergreen Physical Therapy clinic in Bay City. Alicia joined Evergreen in order to practice physical therapy in an environment that embodies her philosophy of treatment. Alicia’s professional interests include orthopedics, neurologic rehabilitation, sports medicine, manual therapy, and aquatic therapy. She also enjoys working with local colleges as a clinical instructor for physical therapy and physical therapy assistant students.
Alicia, her husband, Jesse, and two children, reside in the Midland area. Her personal interests include running, basketball, volleyball and spending time with her family.
Jessica Morgan, MSPT graduated from Central Michigan University in 1997 with a Bachelor of Science in Exercise Physiology, and in 2001 with a Master of Science in Physical Therapy. She is currently a member of the American Physical Therapy Assocation and the Michigan Physical Therapy Association.
Jessica has been practicing in the western and Mid-Michigan area for several years, and is currently the director of the Big Rapids Clinic. Jessica joined Evergreen in order to practice physical therapy in an environment that embodies her philosophy of treatment. Jessica's professional interests include orthopedics, sports medicine, geriatric care, and aquatic therapy. She has also been active with pre-season conditioning programs for area sports teams.
Jessica, her husband, Scott, and two daughters, Samantha and Rebecca, reside in Sears. Her personal interests include snowshoeing, biking, and coaching basketball and softball.
Trisha McMillan, MSPT graduated from Central Michigan University in 1995 with a Bachelor of Science in Exercise Physiology, and in 1998 with a Master of Science in Physical Therapy. She has been practicing in the mid-Michigan area for eleven years and is currently the director of Evergreen Physical Therapy in Clare. She joined the Evergreen team in 2005 in order to practice physical therapy in an atmosphere that embodies her philosophy of treatment. She thoroughly enjoys the “small town” qualities and quaintness that Evergreen provides the community.
Trisha’s professional interests include orthopedics, sports medicine, and geriatric care. The staff has been active with Clare High School in providing pre-season conditioning programs and rehabilitation for area sports teams. Trisha, her husband, Mitch, and their two children, Cole and Addison, reside in Clare. Her personal interests include outdoor activities, traveling, and spending time with her family.
Tim Axtell MS, PT, OMPT, ATC graduated from Ferris State University in 1987 with a Bachelors of Science degree in Applied Biology. In 1998, he graduated from Central Michigan University with a Masters of Science degree in Physical Therapy. Tim has also completed a two-year Post Graduate residency in Orthopedic Manual Physical Therapy at Oakland University. Tim is an active member in the American Physical Therapy Association, and the Michigan Physical Therapy Association.
Tim has been practicing physical therapy in mid-Michigan since 1998 and is currently the director of Evergreen Physical Therapy in Gladwin. Tim joined Evergreen Physical Therapy in order to practice physical therapy in an enriching and fulfilling environment that embodies his treatment philosophy. Tim’s professional interests include orthopedic and manual physical therapy. He has an extensive background in outpatient orthopedic, sports medicine, and industrial rehabilitation.
Tim, his wife Lori, son Gareth and daughter Gabrielle reside in Midland. His interests include skeet shooting and researching Civil War Battlefields.
Mitch McMillan, MS, PT graduated from Central Michigan University in 1997 with a Bachelor of Science in Exercise Physiology and in 1999 with a Master of Science degree in Physical Therapy. He was a member of two Honors Fraternities and is currently a member of the American Physical Therapy Association and the Michigan Physical Therapy Association.
Mitch has been practicing in the Mid-Michigan area for the past several years and is currectly the Director of the Evergreen Physical Therapy Clinic in Harrison. Mitch is happy to join the Evergreen team to offer his philosophy of treatment in a rural community setting. Mitch's professional interests include orthopedics, geriatric, recovery from back injury and sports medicine.
Mitch and his wife, Trisha, reside in the Clare area. His personal interests include outdoor activities, traveling and dog training.
Tammy Thompson, MPT, ATC graduated from Central Michigan University in 1991 with a Bachelor of Science, majoring in Sports Medicine, and from the University of Michigan-Flint in 1996 with a Master's degree in Physical Therapy. She is currently a member of the American Physical Therapy Association, the National Athletic Trainers Association and is a Certified Pilates Instructor.
Tammy has been practicing in the Mid-Michigan area for 12 years, and is the director of the Evergreen Physical Therapy in Midland. Tammy joined Evergreen in order to practice physical therapy in an environment that emobodies her philosophy of treatment. Her professional interests include orthopedics, sports medicine, industrial rehabilitation, manual therapy and rehab for local athletic teams and clubs. She has also been active with pre-season conditioning programs for area sports' teams.
Tammy and her daughter, Tegan, reside in Midland. Her personal interests include running, weightlifting, volleyball, Pilates and watersports.
